Information We May Collect
This may include your name, contact details, date of birth, appointment information, medical and psychiatric history, medications, treatment information, payment information and other information reasonably necessary to provide care.
Our website may also collect limited technical information such as browser type, device type, IP address, pages visited and general website usage information.
Treatment, Payment & Operations
We may use and share relevant health information for treatment, care coordination, payment where applicable, and healthcare operations such as practice administration, quality improvement and provider management.
Appointments & Communications
We may contact you about appointments, intake requirements, treatment-related information, follow-up care and administrative matters relating to your care.
Public Health, Safety & Legal Requirements
We may disclose information when permitted or required by law, including certain public-health, safety, oversight, legal, court or law-enforcement situations.
Additional Privacy Protections
Mental Health & Psychotherapy Notes
Psychiatric and behavioral health information may receive additional privacy protection. Most uses and disclosures of psychotherapy notes require written authorization, except where disclosure is specifically permitted by law.
Substance Use Disorder Records
Certain substance-use-disorder records may be protected by 42 CFR Part 2. To the extent House Call Psychiatry creates or maintains Part 2 records, those records will be used or disclosed only as permitted by applicable law.
More Protective Laws
Where New Jersey or federal law provides greater privacy protection than HIPAA, House Call Psychiatry will follow the more protective applicable requirement.
Uses Requiring Written Permission
Except where otherwise permitted or required by law, written authorization may be required for most psychotherapy-note disclosures, certain marketing uses, sale of protected health information and other uses requiring authorization. You may revoke an authorization in writing, subject to applicable law.
Your Privacy Rights
- Ask to inspect or receive a copy of your health information.
- Ask us to correct information you believe is incorrect or incomplete.
- Request confidential communications.
- Ask us to restrict certain uses or disclosures.
- Request an accounting of certain disclosures.
- Request a paper copy of this Notice.
- Have an authorized representative act for you where permitted by law.
- File a privacy complaint without retaliation.
- If you pay fully out-of-pocket for a service, you may request certain restrictions on disclosure to your health insurer, subject to applicable law.
Our Responsibilities

- Follow the duties and privacy practices described in the current Notice.
- Notify affected individuals when required if a reportable breach occurs.

No website, email system, internet transmission or electronic storage method can be guaranteed to be completely secure.
Website, Forms, Cookies & Third-Party Services
Please do not submit detailed medical records, extensive psychiatric history, Social Security numbers or other unnecessary highly sensitive information through a general website contact or appointment form.
The website may use cookies or similar technologies for essential functionality, security, performance measurement or analytics. House Call Psychiatry does not intentionally permit advertising or analytics tools to receive PHI in a manner prohibited by applicable law.
Third-party services may be used for functions such as secure forms, electronic health records, scheduling, billing, communications, storage and administration. Where a service provider is a HIPAA Business Associate and handles PHI on our behalf, appropriate protections and agreements are used where required.
